Here you will find all of the forms that you need for pageant weekend! Simply find your age division and click on each form for each event that you are competing in. Remember EVERY contestant is required to bring the following forms: Emcee Card, Resume Form, and Community Involvement Form.
Am I required to fill out all the forms listed for my age division? No. You are only required to fill out the THREE REQUIRED forms – Emcee Card, Resume Form and Community Involvement Form. However, you must also fill out the cards/forms corresponding to any OPTIONAL contests in which you are competing. You will bring all emcee cards/forms with you to pageant check-in on pageant weekend. Please do NOT mail your cards/forms to the pageant office. Make sure your cards a filled out before you arrive on pageant weekend.
We will not have printers at pageant check-in, please have all your forms and cards completed, printed out in color and ready to turn in at pageant check-in. Print all forms and cards in color.
PRINTING GUIDELINES
Your pageant paperwork are to be printed either on standard paper or cardstock. Please see the ‘Paper Type’ column for guidance on which forms need to be printed on what types of paper. All paper, whether standard or cardstock, should be 8.5×11″ and white.
The standard office copy paper weight is most commonly called “20 lb. bond.” Cardstock is typically classified by weight, ranging from around 65 lbs (thinnest cardstock) to 110 lbs (thickest cardstock). Any weight of cardstock is acceptable; 65lbs tends to be the most popular. Visit your local office supply store with any questions you may have regarding paper type.
Remember that all cards and forms should be printed in color as the paperwork is color-coded by division.
FILLING OUT THE FORMS
These forms are PDFs with text boxes ready for you to type your responses into. It is easiest to download to a computer and complete that way. However, you may also be able to fill out the PDF from your phone through a free app such as Adobe Fill & Sign, available on iPhone and Android phones.
IMPORTANT: Do not just click on the link — make sure you right click and save each file to your device before working on the form.
